Our HIPAA APPROVED Clinic will help each patient to Retrieve Medical Records if needed to Qualify/RE-Qualify At our Lansing Walk-In location you will sit down with our trained MEDICAL staff for a Counsultation/Pre-Qualification. We will assist you in retrieving pertinent medical information FAST to see if you may qualify for certification.  Patients are welcome to download forms needed to qualify from this page by clicking on all three highlighted links (above) labeled HIPAA, Medical Records and Pre-Qualify. Simply fax, send by mail or email all three forms in for Pre-Qualification and one of our staff members will contact you quickly. All qualifying patients are scheduled with a physician for a Counsultation and brief Medical Examination.

All applications and copies are provided by our clinic for our patients and caregivers. After receiving our physicians approval, every patient is consulted on the Michigan Medical Marijuana Registry Application. We assist you throughout Your Healthy Choice with State Change Forms, Work or Court related issues, Grow Shops, Compassion Clubs, Workshops and Networking.
